This page explains what this website collects and what happens to it. It is written to be read rather than to be impenetrable.
What this site collects. This site carries no contact form. Nothing you type into the on page assessment is transmitted anywhere: it runs entirely in your browser, it is not saved, and it is gone when you close the tab. No account is created and no card details are ever requested.
Telephone calls. If you call the number on this site, the practice holds your details in order to arrange and provide care, in the same way any medical practice does. Calls may be routed through a tracking number so the practice can tell which of its listings people found. That records the fact and origin of a call, not its content.
Analytics and cookies. This site sets no advertising cookies and runs no cross site tracking. Standard server logs record the usual technical information, including IP address, browser and the page requested, which is used to keep the site working.
Third parties. Typefaces are served by Google Fonts and two JavaScript libraries are served from a public CDN. Requesting those files reveals your IP address to those providers, which is true of most of the web.
Patient information. Anything you share once you are a patient is protected health information and is handled under HIPAA and Florida law by the practice, separately from this website. The practice's own notice of privacy practices governs that and is available at the office.
Your choices. You can ask what the practice holds about you, ask for it to be corrected, or ask for it to be deleted where the law permits. Call (813) 749-7200.
Children. This site is not directed at children under 13 and does not knowingly collect their information.
Changes. If this policy changes the date at the top of the page changes with it.
